You have the right to:
  • Request and receive copies of your records
  • Receive information about the counseling process and your treatment
  • File a complaint if you believe your rights have been violated

Requesting Your Records

You may request a copy of your records at any time.

Written requests may be submitted through the secure client portal, via email at [email protected], or in writing.

In your written request please include:

  • Name, date of birth, mailing address of where you would like records sent, and dates of services you are requesting the records from.

If you haven’t received a reply, please call me at 469-888-1896 and leave a voice message including the information requested above.
